Palliative Care

Backed by a team of Palliative Care specialist nurses, Focus Healthcare looks after the palliative care needs of over 700 people in a year. About 20% of these clients die at home with the help of their families. Focus Healthcare clinicians liaise with GPs, Specialist Palliative Care Teams, Metropolitan Domiciliary Care and other community care organisations.

Exposure to the range of needs this client population presents with, means Focus Healthcare clinicians are not only sensitive to the needs of palliative care clients and their families but have highly developed technical and personal skills in:

• Pain management
• Symptom control, nausea, fear, anxiety, fever etc
• Nutritional management
• Continence care (incontinence as well as constipation)
• Intravenous and enteral
• Bereavement support
• Support, education and advice to carers
• Personal hygiene care

Palliative care is planned following a careful assessment which takes into account the involvement and contributions of everyone involved. Care is taken to involve the client, explaining options for treatment, pain and symptom control and home and hospice based respite. Focus Healthcare can provide care including overnight visits.

How to receive palliative care from Focus Healthcare

Any person needing palliative care can receive it from Focus Healthcare. All that is required is a telephone call to 1300 363 262 (24 hours / day). Our contact centre personnel will process the referral promptly and can advise you of charges and options for payment and rebates (e.g. from health funds or the Department of Veterans Affairs (War Veterans).
Once a referral is received a ‘key’ nurse will be assigned to your care. This nurse is responsible for coordination of the services you receive from Focus and for communication with other people who may be involved in your care e.g. your GP, Specialist Palliative Care Team, oncology specialists, and your family.

Charges

Private health insurance, Workcover or motor vehicle 3rd party compensation, Department of Veterans Affairs or hospital discharge/avoidance schemes will cover all or part of the cost of wound care provided by Focus Healthcare Nurses.
