- Clients with varying degrees of dementia/confusion who need nursing care of other health conditions such as diabetes, incontinence and leg ulcers.
- Clients with nursing needs around endocrine, cardiac, circulatory, respiratory or urological conditions, or who need palliative care: with serious psychiatric co-morbidities such as schizophrenia, bipolar affective disorder or depression.
- Clients whose capacity to manage their own medication is limited.
Dementia and mental health care is based on a sensitive and careful assessment of the client. The client is involved as far as possible, with special attention given to strategies to minimise the impact of the condition on clients and carers alike.
How to receive care from Focus Healthcare
Any person needing dementia or mental health care can receive it from Focus Healthcare. All that is required is a telephone call to 1300 363 262 (24 hours / day). Our contact centre personnel will process the referral promptly and can advise you of charges and options for payment and rebates (e.g. from health funds or the Department of Veterans Affairs (War Veterans).
Once a referral is received a ‘key’ nurse will be assigned to your care. This nurse is responsible for coordination of the services you received from Focus Healthcare and for communication with other people who may be involved in your care e.g. your GP, specialists, carers and your family.
Charges
Private health insurance, Workcover or motor vehicle 3rd party compensation, Department of Veterans Affairs or hospital discharge/avoidance schemes will cover all or part of the cost of wound care provided by Focus Healthcare Nurses.
