Health Assessments
Health Assessments are part of a strategy for keeping older Australians healthy at home. Registered Nurses highly experienced in clinical assessment follow best practice protocols and guidelines to complete assessments – in conjunction with each client’s own doctor. Priority is given to ensuring that assessments are booked for times when clients feel comfortable and unrushed. Health Assessment nurses take care to involve clients in the assessment process and advise them on the need for all urgent and non-urgent follow up.
A Registered Nurse from Focus Healthcare undertakes these assessments in the client’s own home, where a thorough safety check is also undertaken. Home visits provide the chance to check the safety of wet areas, kitchens, steps, stairs, floor safety and access to outside of the building. The assessment is an opportunity for clients to discuss any health issues of concern to them with a nurse who is able to provide advice and education. Issues may include: continence, diabetic/dietary advice, hearing, dental, eyesight, physical limitations, social wellbeing, transport, cleaning and general health issues. The nurse will review any medications taken to ensure correct compliance and storage.
A physical examination can be attended to - with the client’s consent. This assessment may include: pulse, blood pressure, height, weight, eye chart, hearing, urine test and foot check. Many successful outcomes have eventuated from these Home Health Assessments.
How to receive a health assessment from Focus Healthcare
Any person wanting a health assessment can receive it from Focus Healthcare. All that is required is a telephone call to 1300 363 262 (24 hours / day). Our contact centre personnel will process the referral promptly and can advise you of charges and options for payment and rebates
Once a referral is received Focus Healthcare assessment nurse will contact you to book a suitable time for the assessment to take place. This nurse is responsible for ensuring that the assessment is complete and comprehensive, that all the findings are documented and for communication with other people who may be involved in your care e.g. your GP or specialist.
Charges
Health Assessments for people over 75 are provided at no cost to clients. For younger people Focus Healthcare will provide a quote on request.
NOTE: Only people over 75 - with a referral from their GP – are eligible for the Medicare rebate for this service
