Diabetes Management

Diabetes is a disease with serious and potentially life threatening complications, affecting an estimated 1.2 million (about 7%) of Australians. Yet research shows that most Australians don’t think diabetes is serious and believe they have a lower risk of developing it that they actually do!

Research also shows that maintaining a healthy weight, being physically active and following a healthy eating plan can reduce a person’s risk of developing Type 2 diabetes by up to 60%. Early diagnosis and treatment, and effective management also reduce the risk of complications.
Focus Healthcare nurses make over 2,000 visits for diabetes care every month.

Focus Healthcare nurses are supported by specialist Diabetes Clinical Nurse Consultants, who assist with the coordination of care. Our clinicians work in partnership with GP’s, Diabetes Specialists and hospitals to assist and support people in the management of their diabetes.

How to receive diabetes management from Focus Healthcare

Any person needing diabetes management can receive it from Focus Healthcare. All that is required is a telephone call to 1300 363 262 (24 hours / day). Our contact centre personnel will process the referral promptly and can advise you of charges and options for payment and rebates (e.g. from health funds or the Department of Veterans Affairs (War Veterans).
Once a referral is received a ‘key’ nurse will be assigned to your care. This nurse is responsible for coordination of the services you received from Focus Healthcare and for communication with other people who may be involved in your care e.g. your GP, endocrinologist, dietician and your family.

Charges

Private health insurance, Workcover or motor vehicle 3rd party compensation, Department of Veterans Affairs or hospital discharge/avoidance schemes will cover all or part of the cost of wound care provided by Focus Healthcare Nurses.
