Community Integration Support

At different time and for different reasons people find themselves isolated from the community they live in. This may be because of the demands of looking after themselves or a partner. It may be because they have moved to a new neighbourhood. It may be because of limited mobility, poor eyesight or hearing or some other infirmity. In any case the isolation can lead to loneliness and inability to use even simple services such as shops, banks, the doctor or even the hairdresser or the movies.

Focus Healthcare Home Support Workers are experienced and qualified (Community Certificate III) with current police checks. They come from the local area and are familiar with the needs of people who need to ‘re-enter’ or link back into the community where they live.

Community integration is based on a careful assessment of client and carer needs including assessment of the interests and needs of each client. Priority is given to identifying and planning for activities which clients enjoy as well as meeting appointments and shopping, banking and other needs. Special attention is given; to ensure that the client is comfortable with the nature and extent of community integration.

How to receive help with community integration from Focus Healthcare

Any person needing help with community integration can receive it from Focus Healthcare. All that is required is a telephone call to 1300 363 262 (24 hours / day). Our contact centre personnel will process the referral promptly and can advise you of any charges, options for payment and any rebates (e.g. as part of hospital discharge planning or from the Department of Veterans Affairs (War Veterans).
Once a referral is received our Home Services Coordinator will assign one or more qualified and experienced Home Support Workers to assist with planning the visits, appointment and outings agreed each week. The coordinator is responsible for supervision of the care you received from Focus Healthcare and for communication with other people who may be involved e.g. your GP and other members of your family.

Charges

Focus’ Home Support Coordinator will prepare a personal quote – based on a mutually agreed ‘program’ for each client. (Health fund, DVA and other rebates may apply). Prices may change if the program is significantly altered.
NOTE: Cost of entry to activities, taxi fares etc are a separate cost to the client.
