Case Management
Case management is used to provide best practice in management of the care and services provide to clients. At Focus Healthcare, case management is used to:
- Match client (and carer) needs with the most appropriate available services
- Advocate for clients
- Provide a common point of contact and communication for all services involved in caring for each client and
- Help clients reach and maintain the highest possible levels of independence and quality of life.
Registered Nurses and Home Support Coordinators with extensive experience in community and primary health care coordinate case management for Focus Healthcare’s clients. Our Home Support Coordinators provide case management for clients whose degree of support does not extend to professional nursing or allied health input e.g. who need help with home support services such as personal care, cleaning, respite and community integration.
Case Management is based on careful and ongoing assessment of each client. Each case manager works with clients (and other people involved in supporting clients) to develop and provide a program which supports the client in their activities of daily living. Priority is given to input and activities / treatments which will help clients regain or improve their capacity to be independent and enjoy a satisfying quality of life.
How to receive case management services from Focus Healthcare
Any person needing case management can receive it from Focus Healthcare. All that is required is a telephone call to 1300 363 262 (24 hours / day). Our contact centre personnel will process the referral promptly and can advise you of charges and options for payment and rebates (e.g. from health funds or the Department of Veterans Affairs (War Veterans).
Once a referral is received a Case Manager will be assigned to you. This person is responsible for coordination of the services you received from Focus and for communication with other people who may be involved in your care e.g. your GP, other health care professionals and your family.
Charges
Private health insurance, Workcover or motor vehicle 3rd party compensation, Department of Veterans Affairs or hospital discharge/avoidance schemes will cover all or part of the cost of wound care provided by Focus Healthcare Nurses.
