Post discharge from Hospital
Most people prefer to return home as soon as possible after a visit to hospital. Hospitals, in turn, work hard to help people with early discharge – as long as support and services are available to make sure that healing and recovery continue and that patients feel safe and comfortable when they return home.
A number of hospitals in Adelaide provide discharge assistance ‘packages’ which pay for a predetermined range of services – designed to make sure patients can make a full recovery at home in the days or weeks following discharge.
Discharge packages combine nursing and other professional support with necessary home support services such as cleaning, help with showering and dressing and assistance with shopping, bill paying and getting to appointments.
An increasing number of hospitals now fund discharge ‘packages’ (of care and support services). These discharge packages are made available to patients who have been assessed by the hospital as needing further care and services once they return home.
Information about the availability of discharge packages can be obtained by asking the hospital discharge coordinator.
